Harnesses are sized by the chest (girth) — the measurement around the widest part of the ribcage, just behind the front legs.

How to measure

Stand your pet up squarely.
Wrap a soft tape around the widest part of the chest, just behind the front legs.
Keep two fingers under the tape so the fit is snug, not tight.
If your pet is between sizes, choose the larger one.

A dog that pulls hard on a lead is usually pulling from its shoulders and chest, which is exactly why a harness clipped at the back gives it more leverage to keep doing so - the lead attachment point ends up behind the direction of the pull rather than working against it. Front-clip designs exist specifically to change that.
